JOB DESCRIPTION 

Provides support for mobile unit activities including support. Responsible for routine and complex transportation services related to driving and transport coordination, in addition to activities related to connecting members to community services and resources. Contributes to overarching strategy to provide quality and cost-effective member care. 

Essential Job Duties 


• Responsible for driving mobile unit to scheduled community events/locations/destinations to support member care. 
• Inspects mobile unit for defects before and after trips and reports any discrepancies indicating mobile unit condition to leadership. 
• Maneuvers mobile unit as needed, includes backing unit in and out, navigating around tight corners, manufacturing areas and high pedestrian traffic. 
• Provides minor vehicle maintenance, (i.e. maintains fluid levels and keeps unit clean inside and out). 
• Performs driving duties in conformance to appropriate safety and security standards. 
• Assesses eligibility for mobile medical services, and provides explanation and instruction on how to access services. 
• Registers member in mobile services electronic medical record (EMR) system (Epic), and schedules services as needed. 
• Runs required reports and transmits discharge summaries to identified providers. 
• Assists members in completing enrollment paperwork for mobile services, and coordinating with enrollment team, program clinicians and providers. 
• Evaluates and resolves issues related to member ability to access services on the mobile medical unit. 
• Educates other agencies about related mobile services and access criteria. 
• Establishes and maintains relationships with community and social resources. 
• Provides technical assistance, guidance and direction staff as needed. 
• Up to 75% local travel may be required (based upon state/contractual requirements and seasonal need to support events). 

Required Qualifications

• At least 1 year of related experience related to transport, customer service, and or health promotion/health education, or equivalent combination of relevant education and experience that demonstrates ability to do the job. 
• Valid and unrestricted driver's license (CDL preferred), and adequate auto insurance for job related travel requirements. 
• Ability to drive a 39’ Class A Winnebago. 
• Thorough knowledge of traffic laws and defensive driving. 
• Ability to operate required tools and equipment. 
• Ability to safely drive various business vehicles. 
• Ability to establish and maintain effective relationships with employees, leaders, members and the general public. 
• Ability to communicate effectively, work independently and make independent judgments which have moderate impacts on the organization. 
• Ability to perform heavy manual tasks under varying weather conditions. 
• Strong verbal and written communication skills. 
• Microsoft Office suite/applicable software program(s) proficiency, and ability to learn new programs. 
• Ability to lift 40 lbs.

• Experience in a health care setting.

Pay Range: $17.14 - $34.88 / HOURLY
