Job Summary

The Nurse Practitioner / Physician Assistant (NP/PA) – MCW is responsible for completing in-home history and physical assessments for enrolled members in health plan products to coordinate a medical plan of care and capture diagnoses for Medicare risk adjustment. The NP/PA will utilize advanced assessment skills to diagnose and manage chronic health conditions to improve member health outcomes. The NP/PA is responsible for completing advanced practice consultations for enrolled members in the various health plan products identified due to medical complexity, increased utilization, palliative care needs, or care team support. The NP/PA will conduct consultations as assigned and utilize health records, internal documentation, and discussions with internal care team staff and outside providers to develop a medical plan of care and collaborative care plans when needed to improve member health outcomes. In addition, the NP/PA will collaborate with members, primary care providers, internal care team staff, pharmacists, specialists, and other community providers to develop member specific medical care plans.

Job Duties

  • Conduct comprehensive in-home history and physical assessments consistent with department process. Identify all active and chronic disease conditions as well as determine all physical, mental, and social needs present at the time of the visit. Obtain vital signs, measure BMI, review pharmacological therapy and conduct a physical examination. 
  • Capture and document annual diagnoses accurately consistent with Medicare risk adjustment.
  • Conduct comprehensive annual medication reviews and follow up, as necessary. 
  • Identify gaps in acute/primary care and chronic disease management. 
  • Identify medical risk and collaborate with members of the care team to mitigate. 
  • Communicate recommendations and collaborate with primary care providers/specialists to ensure clinically relevant treatments are explored, if not already in place. 
  • Communicate recommendations and collaborate with other providers to confirm suspected, but not yet identified/coded, diagnoses. 
  • Develop a medical plan of care in conjunction with primary care provider/specialists as dictated by member need and assessment/consultation findings. 
  • Collaborate with manager and other clinicians in the department to assure effective patient care consistent with quality indicators for patients cared for in the designated service. 
  • Educate and counsel members and family on any conditions identified during assessment and screening procedures. 
  • Provide quality member centered care.
  • Ensure timely appointments and documentation per department procedure. 
  • Provide consultation, as needed, for medically complex members, high utilization, or subject-matter expertise (e.g., Palliative Care, Dementia, Psychiatric). 
  • Other duties as assigned.

Job Qualifications

Required Education: 

Required to be a Nurse Practitioner or Physician Assistant as outlined below:

  • Nurse Practitioner Positions: 
    • Graduate of an accredited school of nursing.
    • Graduate of an accredited Nurse Practitioner Program
  • Physician Assistant Positions:
    • Bachelor's degree in Physician Assistant studies of an accredited school.

Required Experience, Knowledge, Skills, and Abilities: 

  • Proficient knowledge and skill using MS Office Products such as Excel, Word, Outlook & PowerPoint. 
  • Strong organizational skills with ability to multitask and prioritize various projects. 
  • Ability to function independently with minimal supervision on a variety of tasks.
  • Ability to organize and prioritize workflow.
  • Communication – ability to provide information effectively with a diverse population.
  • Managing Priorities/Deadlines – Ability to maintain schedules, meet deadlines and manage multiple projects.
  • Problem-Solving Skills – Ability to think critically and be solution-oriented in a fast-paced environment.
  • Adaptability – Ability to manage change, deal with situations as they arise and work independently or as part of a team. 
  • Teamwork – Ability to work as a team participate productively while also managing independent contributing duties and responsibilities.
  • Interpersonal Skills – Ability to work with others and relate to client(s)/member(s). Able to establish rapport and build effective relationships at various levels in a diverse setting.

Required Licensure or Certification: 

  • Current national certification as Nurse Practitioner through ANCC or AANP in Geriatrics, Adult-Geriatric, or Family practice or eligibility to obtain within reasonable timeframe and current and unrestricted State of Wisconsin Advanced Practice Nurse Prescriber license and Drug Enforcement Administration registration required.

OR

  • Current State certification as a PA, or eligibility to obtain within reasonable timeframe, current and unrestricted State of Wisconsin Physician Assistant license and Drug Enforcement Administration registration required.
  • Current unrestricted driver’s license

Preferred Qualifications:

  • Previous experience as a Registered Nurse, Nurse Practitioner or Physician Assistant as well as experience with the geriatric and/or disabled populations.
  • Two or more years of clinical experience.

Travel Requirements:

Driving:  50 – 60%
